The deadline to file claims against your 2009 health FSA is April 30, 2010. 2009 Dependent Care claims must be filed by March 31, 2010.
Claims incurred through March 15, 2010 can be claimed against any remaining 2009 funds. If you are planning on filing 2010 expenses against your 2009 funds DO NOT use the debit card from Discovery Benefits for the charges. The debit card accesses 2010 funds.
Use the Flex Claim Form Health Care to submit claims for reimbursement for health care expenses.
Use the Flex Claim Form Dependent Care to submit claims for reimbursement for dependent day care expenses.
